Because the non-lethal system is pretty finicky. Everyone can be taken out non-lethally, but you need to be careful not to hurt them after they get knocked out. One swing too much can kill them off instead.
That came in 1.2 because people kept killing pyschos by accident. Hack them using Short Circuit or Overheat. EMP grenades. Shoot their legs. I use the Quick Hack Cripple Movement then walk behind them and grab them. From there use Non-lethal Takedown option and done. Works even if they are at full health.
Swinging with fists works, but if you punch just once more, that's it. Get a nonlethal mod for one of your weapons. 100% better. Uses Blunts weapons like bats or look for weapons that say non-lethal on them. Also yes fists count as non-lethal only if you don’t do the finishing move when V punch the opponent in the neck.

non-lethal sucks and is not incentivized. I'm 40 hours in, focusing on quickhacking and stealth, and it really feels like the game could give a fuck about this build. There's no reward or satisfaction. Non-lethal weapons kill, quickhacks cant be used at critical moments, enemies dont wake up from knock outs, etc.
